Patients visit their GPs for a range of different reasons and sometimes these issues can be caused by non-medical matters such as loneliness, anxiety, unemployment, illness or debt.

Social Prescribers work in partnership with GP surgeries and can help people to access appropriate support in the community to help them make positive changes to your personal wellbeing.

Bex works alongside Baddow Village Surgery  for a company called my mhealth and is one of our practice Digital Health Advisers.

my mhealth is a clinically led software company that develops and deploys Apps which help patients and clinicians to manage long-term conditions better. Since their launch, my mhealth have worked collaboratively with 100s of CCGs, NHS Acute Trusts, GP practices and community providers to digitally transform the way they provide care and interact with individuals with long-term conditions.

Since the start of Bex’s role in June 2020, There have been over 6000 patients registered for the my mhealth Apps across Mid and South Essex, which is expected to grow substantially with support from
Baddow Village Surgery.

Patients can self-refer if they have one of the following conditions: COPD, Diabetes ,Asthma or Heart Failure.
